How to complete application forms

Click on the desired application form.

Once the form opens, complete all required information.

Save the completed form to your device.

You now have two options.

You can send the completed form through your regular

email service to mlnuhc@sasktel.net 

OR

Navigate to the Contact Us section of this website.

Enter your information in the Contact box and then click Add Attachment. A new window will open and allow you to locate the completed form on your device. Once found, click on the form and it will attach to the message.

Now click the send button.
